Evaluation Forms
To effectively measure your employees' performance, this form with "Manage performance" permissions, provide a structured framework that encapsulates all pertinent details.
- From the home page, click on 'Performance'.
- Then, select 'Evaluation Form'> 'Create New'
- A new page will appear, presenting all the relevant fields for the evaluation to be completed.
What are the steps for creating the evaluation form?
Form Details:
- The initial stage of the form is 'Form Details', and progression to subsequent stages is restricted until this section is completed. Here, you must input the form name in both English and Arabic, and provide any descriptions pertaining to the evaluation.

Evaluation Period:
1. Calendar Year
- The "End" field, which determines the evaluation form's expiration date in the calendar year model, is automatically filled in by the system once the starting month is entered, considering that a calendar year is calculated as 12 months from the start date, and not based on the ending of the current calendar year’s conclusion.
- Once the recurrence and start date are set, the system automatically generates the evaluation form(s) along with suggested dates for when the evaluation form will be active—allowing the reviewer to conduct the evaluation. However, these dates can be adjusted as needed.
2. Contract period: Select this type when the evaluation is designated for employees whose contracts are nearing their conclusion within this quarter year. Similar to the probation period type, ensure that the start and submission times of the evaluation are configured to coincide with the end of the contract period.
Please note that if employees with indefinite contracts (open contracts) are added to this form, they will be evaluated based on the start date of their contracts. Employees whose contracts do not expire within the current calendar (in which the evaluation form is created) year are not eligible for this evaluation form.
Example: If the start date of employee Jamila’s open-ended contract is January 5, 2024, her evaluation date in the end of contract evaluation form for 2025 will also be tied to January 5. This applies to all subsequent years for evaluating the end of contract form of this employee. Learn more about end of contract evaluation forms.
3.Probation period: for this type to enable the evaluation form specifically for employees who are currently in their probation periods. You can set the start and submission times of the evaluation to align with the duration of the probation period. In this case, the employees available for addition to this evaluation form are only those who are still in the probation period on the system.
Note:
If the employee is not marked as being in the probation period at this stage of creating their profile, the admin will need to edit their profile and create a probation period for them to appear later in the probation period form.
Please note that:
If the probation period for an employee is extended in the system, the evaluation date in the evaluation form (probation period) assigned to the employee will be updated to align with the new probation period dates.
How can the probation period duration be updated for employees?
4. Fixed duration:
- Any employee can be added to it (including those in the probation period).
